Oak Valley Health (OVH), whose mission is honored to care, is dedicated to providing training programs that will supply the necessary tools for managing a variety of patient’s healthcare needs. The training courses are taught by instructors certified by external organizations as having met requisite skills and training components.
In consideration of being permitted to participate in the training course, I, for myself, my personal representative, assigns, heirs, and next of kin:
1. Fully understand that there are certain risks of injury involved by participating in the training courses, especially activities involving skill practice and testing, including but not limited to risks of illness, injury including serious bodily injury or death. I agree to waive and relinquish any and all claims I may have arising out of, or connected with, said course, whether they relate to injuries to myself or others.
2. Fully understand that the training course provides knowledge. I further agree to hold harmless Oak Valley Health for all injuries, be them physical, fiscal or otherwise, that I may inflict upon others as a result of the course instruction.
Billing and payments
Oak Valley Health will charge the following fees through the Portal: course registration fees and other dues as established and communicated from time to time. You shall pay all fees or charges to your account in accordance with the fees, charges, and billing terms in effect at the time a fee or charge is due and payable. We reserve the right to change products and product pricing at any time. All issues regarding course cancellations and reimbursement of course fees are OVH’s responsibility.
Privacy
In order to provide you with the course instruction, you may have to access other related services through links to third-party websites. Such websites are created by organizations outside OVH and may include, but are not limited to the Heart and Stroke Foundation of Canada. These links are provided solely as a convenience to you and not as an endorsement by OVH of any third-party website and the content thereof, unless specifically stated; as such OVH is not responsible for the content of linked third-party sites or third-party advertisements and does not make any representations regarding their content or accuracy.
If you access or use third-party services, be advised that your access and use of such other services are governed solely by the terms and conditions of such other services. Your use of third-party sites and services is at your own risk. You irrevocably waive any claim against Oak Valley Health with respect to such other sites and services.
